Patient Information
Ford's Foot Performance operates by appointment schedules and requests that you call to schedule an appointment in advance. Your appointment length will usually take between 30-60 minutes. It is our belief that quality care takes time; therefore, we will never compromise care by rushing our patients.

What to Bring
If you are a new patient or if it has been over a year since your last visit, pre-registration is required.
New patients please bring:
Driver's License
Insurance Card (If you plan to use insurance)
Physician's prescription (when required for medical services)
Shoes you wear on a regular basis
Any current foot orthotics
Intake Forms (located at the bottom of this page)
Patients returning for their fitting please bring:
Shoes that were discussed on the initial visit (Click here to find information about this visit)
Patients returning for an adjustment or refurbishment please bring:
Your orthotics
Shoes you have worn with the orthotics
The box containing your foot models
